Send an email to support@not-forgotten.com and provide the following information:
• Email address you used to register your memorial account.
• Title of the Online Memorial you have created and wanted to be linked to your UNIQUE QR Code.
• The link or an image of your UNIQUE QR Code (from the QR Label Stickers that came with your Frame).
